Explaining the "Hungerstat" – Read This Before Considering a Gastric Bypass
When I have an obese patient in his 300 pound state, he often comes to me for advice and to clear him for gastric bypass surgery. Some insurers require these patients to undergo 6 months worth of dietary and psychological counseling prior to surgery, some require a primary care physician like me to care for them during the perioperative period (the time surrounding surgery) for all reasons. However with my obese patients I hate giving this clearance because I have seen many of my patients end up with complications including death. I had one patient die of sepsis after surgery. One patient ended up going in the opposite direction from obesity and becoming bulimic. One patient is constantly receiving blood transfusions due to iron deficiency anemia and the other is B12 deficient because the part of the stomach that absorbs vitamin B12 is now blocked from absorption. And I have countless patients who have lost weight… only to gain it back. For every successful patient after gastric bypass, I have 8-9 other patients who have major complications throughout their lives because of the permanent changes made to their stomachs.
But I have fewer and fewer patients experiencing these issues, yet with weight loss similar to gastric bypass. This is because of my new understanding of the hunger thermostat in our body called “hungerstat”. I offer my patients an alternative weight loss method that is free and does not require surgery, and most of them have avoided bypass with this weight loss system. Let me explain the state of hunger before I go on this diet.
Hunger is in all of us and it can be controlled. Surgery and gastric bypass can forcefully alter our satiety levels. That’s a given or we wouldn’t be offering such a dangerous option for treatment to our sickest patients. However, Alvin Lewis, MD, MPH, and I are using our bodies’ natural appetite control system to steer our patients away from gastric bypass, allowing them to lose weight at the same rate without surgery.
How should we do this? We use our body’s natural systems to lose weight and maintain health. When we were hunter-gatherers before the Ice Age, when famines or winters came and food was scarce to hunt or gather, our bodies would starve for 3 days. After 3 days, the hunger goes away because the body goes into catabolic mode: this is the mode that allows fat to be burned. When you eat 10 or less meals a day, this mode turns on automatically after 3 days.
So specifically, if you eat five bites twice a day, after 3 days of hunger and fatigue, your body will turn off the feeling of hunger and at least feel satisfied with food! The body is satisfied because it starts burning fat to keep you alive. How we designed it. Your hunger thermostat, or hunger, tells the body that it is no longer hungry.
Look at the children in those poor countries, those ads asking you to sponsor them for a few rupees a day. When you see them, they are sleeping. Although they are malnourished, believe it or not, they are not starving. They look terrible, don’t get me wrong, to be in that situation. But nature was good enough to design us not to feel that hunger after 3 days so we don’t suffer. Those children are using their fat reserves to survive and their appetites are completely turned off.
So the surprising thing is that if you eat five bites twice a day, you will not feel hungry at all in 3 days. Sounds like you have gastric bypass.
However here is the second amazing thing. After 14 days of easily taking just 5 bites twice a day, the stomach will reduce naturally. In other words, after 14 days of eating just 5 bites twice a day, the stomach will shrink to a point where eating more than 5 will make you feel intensely full. Your thermostat for hunger drops to the point where you are satisfied with a small amount of food. Your portion sizes, from that point forward, are naturally regulated as your stomach goes through gastric bypass. My patients on this diet are in control for the rest of their lives because they have naturally controlled their appetite so they no longer feel the need to go to the buffet with a full plate for 3 visits, they no longer overeat, and they no longer have the ability to overeat in their stomachs (they blow it all up and unless consuming large amounts of food and increasing the capacity of their newly shrunken stomachs).
If you’ve ever seen a friend after surgery, their appetite won’t allow them to eat more than a few bites from their plates. Hospitals constantly have people with full plates on the table, even if the surgeon agrees to eat as much as the person wants. But many people cannot regain their appetite after a few days of not eating due to abdominal surgery or abdominal conditions. These people are having the same exact effects on their appetite. So the question is…does gastric bypass really help people lose weight or does the post-surgery period just cause less appetite?
So with cutting back on the intense portion and eating only 5 bites twice a day, in 3 days you will feel no hunger and in 14 days you will feel satisfied after only 5 bites. Sounds like a guy with gastric bypass doesn’t it? But instead, you’re using your body’s natural appetite to make the same exact changes.
After gastric bypass, you eat 3 bites 3 times a day. When my patients come to me for a pre-surgical consultation, I refer them to Dr. Alvin put Lewis on his “Five Bite Diet” and he lost the same amount of weight, had the same effects on his appetite, and avoided surgery altogether. On this diet, they lose 1 pound per day. And best of all, the food costs nothing. You save from surgical costs, and costs to your body as you avoid life-threatening surgery.
Many surgeons are starting to get angry with me because when they send these patients to me for pre-operative counseling, they rarely come back because this diet allowed them to lose 1 pound a day as if they had a bypass. They finally see weight loss success when they feel their only option is surgery. And he smiles that he has avoided such a heavy danger. And it’s thanks to this new knowledge in hunger.
