Routine Annual Physical – Not Worth the Time or Money
A recent article in the Archives of Internal Medicine on September 24 found that annual physicals may not be the most beneficial. Although these types of visits account for 1 in 12 doctor’s office visits, the study found:
* Only 19.9 percent of eight preventive services were provided at these exams as opposed to other types of doctor visits.
* Only 8.8% of weight loss counseling occurred during these visits.
*Mammograms, cholesterol testing, and counseling patients to quit smoking occurred in half of the encounters.
* People in the Northeast are 60 percent more likely to have a normal body type than those in the West. (Note a recent Health Affairs study also found that per capita health care costs are higher in the Northeast).
This is not surprising. A typical physical property value is no more helpful in finding problems than having your mechanic check up on your car, which is running fine, every year otherwise. For decades, there was no scientific evidence that the body every year helped save lives. For those who are healthy, do not take regular medications for blood pressure, diabetes, cholesterol, asthma, emphysema, or other potentially dangerous health problems, there is little benefit to “checking out.”
However, this should not mean that people should also avoid doctors. Just like your car has a maintenance light that indicates it needs service, patients must have tests that screen for various health problems and cancers based on age and gender. For example, all women without a family history of breast cancer should have a mammogram starting at age 40 and repeat every two years. Although this test can be done during the annual routine, the test can be done at another visit in the future. The study showed that many of these preventive interventions occurred in other office meetings.
Find out what tests you should have at the US Preventive Services Task Force website (http://epss.ahrq.gov/ePSS/search.jsp). It’s a good start, but not the last word. Check out other organizations like the American Cancer Society, the American Heart Association, and the American Diabetes Association for their recommendations as well.
If you have an annual physical aid that reminds you that certain tests should be done to stay healthy, continue the practice. Otherwise, save your time and money. Find out what tests you need based on your age and get them done – no annual physicals are required.
